• When we use the term "PHOTO" it refers to photos or documents or anything that will fit under glass.
• Custom Charms are made from "photos" that you supply. Memory Charms are pre-made items (as-is).
• Custom orders can NOT be returned.
• When submitting hair, fabric or loose items please put in a baggie and write on the baggie.
• Because we can not take responsibility for lost photographs or documents we ask that you send copies instead of originals. You must write your name, telephone number and item number on each photograph / document.
• When ordering more than one item, please place your photos / documents in separate small envelopes with your name, telephone number and item number on the outside of each envelope.
• When marking photographs / documents please allow ink to dry thoroughly. Do not put face of another photograph or document on top of ink.
• Photographs of any size can be used. Smaller is better, each time a photo is reduced it looses clarity.
  Color and tones of original photographs may vary slightly when reduced. Darkest areas can occasionally reflect some glare.
• Irregularities in the shade, shape and texture of each piece are natural occurrences, they enhance their beauty and uniqueness.
• All chains are solid sterling silver. All photographs are under glass (unless otherwise indicated) and encased in silver. To polish, use a jewelry cloth available at any fine jewelry store.
• Photographs can be ordered in original color or, for an aged effect, in black & white, or sepiatone.
• Gift boxes include decorative gold wire ribbon and a small note card.
• Please allow 2 - 2 1/2 weeks for delivery. 4 weeks delivery before major holidays.
• All WellsWare™ charms are proudly handmade in the USA.

• Individual custom & memory chars can ONLY be ordered after first purchasing a custom bracelet.
• Only #2 size charms can be used on dangling bracelets.

• 1. GO TO PRINTABLE ORDER FORM  print it and use the back button in your browser to return to this page. Also print this page.
• 2. Fill in Name, Address, Phone & Payment portion of Order Form.
• 3. Fill in chosen Item #.
• 4. Fill in brief description of item.
• 5. Fill in total number of "Photos" being submitted for that specific item.
• 6. Where given the choice (Necklaces) , fill in whether you would prefer your charms permanently Fixed in one spot or if you prefer your charms Loose. Loose charms move freely.
• 7. If your item has more than 2 custom charms Do Not fill in size or color boxes.
• 8. If you would like a gift box mark area with the letter 'Y'.
  If you Do Not want a gift box mark area with an 'N'.
• 9. If your item is an Ornament or Frame fill in your Ribbon Choice, ie. 'Gold' and your Charm Choice, ie. 'Crystal'.
• 10. Fill in quantity. To order more than one of the same item fill in quantity desired.
• 11. Fill in price of item.
• 12. On the back of each photo you are sending fill out the following:Your Name, Phone #, Item #, Shape/Size code, Color choice ( color, black & white, sepiatone), Charm # (1,2,3...),Indicate Front or Back. *Photo 1 Front and Photo 1 Back will make up one single charm. If you are sending loose items please put into a baggie and fill out same information for photos on the outside of the baggie. These items are seen from the Front as well as the Back.
• 13. On an envelope write your Name, Phone # and Item #.
• 14. Place all 'Photos' in corresponding envelope and seal.
• 15. When ordering more than one item, please place your photos / documents in separate small envelopes with your name, telephone number and item number on the outside of each envelope.
• 16. If ordering Additional Custom Charms place additional 'Photos' in separate envelopes clearly marked Additional Custom Charms. Use new line on Order Form for each Additional Charm and repeat step 12.
• 17. If Ordering an Additional Memory Charm , on a new line on Order Form simply fill in item #, Description, Fixed or loose when given the choice, quantity and price.
• 18. Total Price if Items.
• 19. Add appropriate Shipping Charge.
• 20. Total cost of Gift Boxes ordered.
• 21. NY residents add 8.625% sales tax of Total Price of Items.
• 22. Fill in Final Total at bottom of form.
